Modifying boot.ini to run in safe mode is unreasonably dangerous. While the system is booting, tap 'F8' (right before you see the Windows XP startup screen) to bring up a boot menu, and choose safe mode from there.
Modifying boot.ini can result in a seriously broken system if done incorrectly (ie: if you remove an extra space, or typo a directive).
Moreover, the problem described doesn't necessarily indicate a virus or spyware infection - it could be that a spammer has found a known-good email address and is using it in the 'From' fields of his/her spam. The legit 'From' frield makes it more believable and is more likely to be accepted, and the spammer never has to see the bounces (because they're delivered to the original poster). This is very common, and there's *nothing* you can do about it.
