important to include the deelivery schedule (as this can be lost in all the gumf, and be argued about)
its usually better practice to agree the payment and workload prior to producing an invoice, then there is no arguing.
otherwise, do as they said above, all you need is a contact address (must include street etc) and all the rest (tel. no & email) PLUS the details about the job, ammount, date and payment terms (28 days...)
No need to even sign it.
