The best way to bill is definately by the hour when in doubt. Offer the client X amount of hours at Y rate so you can pay Z bills! Oh, and let them know in advance that they will be billed at an hourly rate of $Y if they request work past the alloted time.
Client doesnt have and suprised glazed look on their face, and they won't pester you to keep changing things if they know it is going to cost them. Or if they do... well money in the bank!
