I use them for coupons, scavenger hunts, and analytics. They produced huge results on this print ad. It really depends on your target audience when using them. In this case it was for female teenagers. Ran in seventeen magazine.

I get very good results actually, I'm able to pinpoint the interest in vehicles, after hours. As far as tracking, of fucking course you can come up with unique URLs and track them, even through Google Analytics, but I'm too lazy, plus the client has central point of tracking through a 3rd party. If they're happy, I'm happy. Not interested in spending the time developing custom URLs and tracking them through GA.
In terms of printing, we use an outside vendor, extremely cheap, due to all the other printing that we do.
Ideal for adhesives and placement on windshields, etc. Perfect for the car industry.

All the QR services direct visitors to their sites first, track it and forward to your site. This is heavily reliant on the third party infrastructure to handle the requests and if they disappear, your links go dead.

Yep, client places them on vehicles. I create them, print them and place them for a dealership. I capture the metrics using a 3rd party tool, actually works quite well.

Scanned a QR code on a business card once, it came up with all the person info and then put that on my contact on my iPhone, which I thought was pretty cool, saved me doing it manually.

I have one on my site that is a Vcard. I have all the info next to it but if someone wants to save my contact, 1 scan and it's done, no need to manually type all the fields. How is that bad?
It is the use people give to stuff that is bad, not the other way around.
QR codes shouldn't replace URL's or any important info, they're just a plus that you can use to facilitate the way you deliver a message.

stil hype. i've never scanned one, personally. get asked quite often if it's worth it to incorporate them into print material.
until phones (if they ever will...which is highly unlikely) incorporate an integrated OS qr code reader, the point of using them is like using banner ads to sell viagra. of course you will get a click-through, but the percentage of return is so minuscule it becomes moot.
