Have you tried talking to your print reps? Let them know what you're trying to achieve, and they may have some existing dies lying around that are appropriate.

For a one off? Die costs'll be high if there's any custom requirements — have you considered DIYing it or getting a design of yours laser cut?

Are you trying to assemble a prototype / one-off or are you trying to find someone who assembles / fabricates these types of boxes?
