Acrobat and Doc Index

  Does anyone know an easy way in Acrobat to have the attached search index open automatically when the PDF is opened?

  Or ... can you create a form with a search box to access it directly? So it more resembles a website search?

  Sorry if this is basic, don't do a lot of Acrobat work.
