I always base deadlines on having everything i need to get started. Basically - hand over the info, i can have it to you in 2 weeks. If you dont hand the info over for 3 weeks, you dont see it for 5.
If its a rush / last minute / unexpected job then they *know* they are putting you out, and a simple 'my production schedule is already full. I can fit in your work, but it means I have to push someone else so I have a rush rate of $X to account for the shuffling of business, customer service, etc it causes me." If youre good they pay you and usually happily. If not they'll find someone else.

150% of standard hourly rate. In extreme cases, 200%.

Damn, I actually was hoping this would be about visiting a client again and and again finding that their shitters were all caked with poo goo, boogers on the walls and the early makings of a glory hole carved with the end of an ice pick.

state the expected content delivery dates clearly, and explain to them that if you don't get the content by X date, the deliverable date will be pushed back by X number of days.
protect yourself with email tracks and copy everyone involved in the project. Don't be afraid to push back on them.
