  We use Codebase, and we have also used Pivotal.

  Pivotal was pretty good, but we now tend to just use Codebase for features/bugs/enhancements and just keep us all up to date using Gobby (collaborative text editor) and by keeping up to date with Git Commits. It's very simple but works really well.
