Sounds like a tricky diecut (especially to register/bleed) and the binding would look atrocious.
That barrel idea is fantastic. I think you should pay Gnash a bazillion for it. If the barrel is lying on it's side it would be extremely easy to diecut and bind that shape, or standing up you could bind the top edge for something different.

Your instincts are sound. those gimmicky die-cuts should be reserved for the childrens section.
perhaps a head-on shot of an old, well used, wooden wine barrel (the kind they crush the grapes in. The wood would give you a nice place to burn in the title - like a stamp. could even have some juice flowing through the slats of wood.
To appease the client you could have some grapes peaking over the top of the barrel - which you could diecute. This way the binding of the book is pretty normal.
