What's the reason for adding noise?

the banding is occurring because your grad is too subtle for the distance between A + B... can only stretch 8%K to 0% so far.
to skip PS step you could rasterize the grad in Illy, and add noise.
but really your grad is probably too light for print.

The problem was worked around by doing the blend in photoshop and placing in Illustrator. Is that the correct procedure? One would think that Illustrator could work?

If you're getting banding it's probably because you're using a mix of Pantone and process, which I've found confuses some programs and ends up in diluted colour, or there isn't enough variation between the two colours for the amount of space being covered.
