Print address book

I want to make a digital copy of a super old address book, which will then be printed out again into a book.
Any ideas on how to accomplish this? Is there any publishing service that allows you to make a book with some kind of easy separation of each letter of the alphabet?
Thanks.

don't know exactly what you're asking... do you want to print the scanned version of the old book, or do you want something that will redo the book automatically?

I'm making a gift for people who are not into using their electronic devices for their addresses. From something like this:
To a modern copy, but it still has to be in book form.
First, I will be re-copying all the information (InDesign > PDF) and keeping separate pages for each letter of the alphabet.
Not sure what to do from that point on. I want a publisher that is able to make the same cut-outs for each letter like the old-school address books. I was hoping that some service like Lulu could do it, but I'm guessing I will have to get the book custom made.
Otherwise I will have to make do with just having page separations for each letter, or make my own tabs.
Any suggestions?
