Looking for Bespoke Jewellry

The missus has a big birthday coming up and there is a need for something special. I was thinking along the lines of a bespoke jewelry (ring probably). I've spent some time scouring google but want to know if anyone here knows of any quality designers/shops/sites that might be more off the beaten track.

there are a lot of jewelry artists on etsy, and they're all willing to do custom stuff. maybe get her something one-of-a-kind?

I did sketches for this guy to design my wife's engagement ring. He sent some back, I approved. It was great. He found the stones, metal to specs and had the setting computer designed. Totally awesome experience:
