How big do these have to be?
I'm getting conflicting specs from a bunch of places, some say 57 x 57px, others say 60 x 60px.
Also, I read somewhere that the app store also needs a 512px x 512px version of it - is that true?

Yes, App store requires the 512 x 512 version that it uses to make several sizes.

Measures 57 x 57 pixels, with 90 degree corners (if the image measures other than this size, iPhone OS scales it)
*Does not have any shine or gloss
*Does not use alpha transparency
