doesn't seem to want to autoformat my docs that don't have an opening and closing <HTML> tag (they are.ctp files which are part of a web app). Is there some setting to get it to format this kind of file?

Can you you just add them, format it and then delete them?

Nope, not that I'm aware of. DW doesn't recognise HTML (or any language) when it's chopped up as includes.
