Just to get a wide opinion in this topic. In what things do you focus to define your baseline height?

Look to the text cap height and then consider east European accents. Tthen increase the leading depending on the line length. For a really long line length, a greater amount of leading should be used.

I try to take it from the smallest element up, ie. the captions and scale from there, but whaddo I know?

Always use 6 for print.
16 for web.

I'm working in InDesign. How should I do if I have a baseline of 12pt and a desire to align text much smaller. In short, I need to have 2 or 3 lines text (smaller) in my baseline of 12pt.

I always set my baseline to my body copy leading, so if it's 10/12pt my basline will be set to 12pt. Obviously everything can't adhere to that so I generally make sure that the first line of any paragraph/heading/outquote sits on the baseline.

if you go into > Text Frame Options > Baseline Options set your first baseline minimum to whatever you've set your grid too and it will snap at the top automatically, very handy.
