Can you recomend a good web app that lets the client see design changes etc via some kind of extranet/web app?

Check the filter, there are loads of options.
One thing you might want to consider is that if you want to control the flow of the presentation without any client screwing around, try WebEx. We use it quite a bit. it's alright.
