alrighty, can anyone recommend a decent (and secure) php / html for that includes an attachment element.

Lately it seems that nothing is invisible to spam-bots, even with captcha and @mangle. I've found that by sticking your form into an iframe and naming the iframe something more-or-less random it's much less visible to the bots.

I think validating the extension of the file and using captcha or similar would do it, right?

It depends on your project requirements.
A guaranteed way to avoid any spam bots is to add the form after document load using Javascript; but that may not possible if no JS compatibility is a must.
