the other option might be to build your own online survey tool and host it yourself and then you won't have an external url..

the link on a page on our website will pop-up a page from our own site, which then displays the questionnaire from the other link...

where is the questionnaire hosted, the url can't be that bad that you need to hide it? I personnally would worry about it esp. if the organisation doing the questionnaire is well respected it may even add legitimacy to it.

It's a security feature showing the domain in all windows now, unless the user manually right clicks and de-selects the 'main' navigation toolbar - which I'm not sure most people even know how to do.

Yes, but I thought pop-up blockers only blocked pop-ups from other sites... adverts and things. Surely you can make a link open a new window?!
