survey tool help/advice

alrighty, i need an online survey tool that enables me to either alter the domain so it looks like it's hosted from my site or can let me place the multi-page survey into my own site.
would prefer it to allow me to sub-domain name it the same was that mailbuild.com (among others) lets me - ie: http://mydomain.createsend.com

already using surveymonkey but i need the URL to be changed so it looks like it's hosted locally or on a sub-domain. survey monkey doesn't offer this (but has been great apart from)

thanks fodcj but that one doesn't offer the URL switch (using the DNS CNAME entry generally) i need. a few do that i've found but the cost isn't cheap for the premium level package.

Why not buy a domain and have it forward to the survey while keeping your domain visible? Not a perfect solution but may do for what you are needing.

the issue is that these surveys are hosted buy the companies online for security reason (https) and unless they offer the DNS CNAME option as a service the survey will always appear with their URL. my client wants the survey to appear as a product of theirs as they have brand market-value. survey monkey's service has been great and the price is excellent but the client is having issue with the name. buying another domain wont change a thing as the survey still exists on the suppliers domain.
