For print; it's still evolving as we just started using this system a few months ago:
Creative (comping stage)
- Proofs
Production (approved by client)
- Proofs
- Final
Support
- Copy
- Finals (final images, logos, etc)
- Fonts
- Images (FPOs, rough 'shopping, etc)
- Working Files (PSDs, AIs)
- Resources (non-copy info like graphs)
- Stock (photog)
- Vendor (anything supplied by the client that will end up in the layout)

i dont get the "concept" folder..wuz in it?

CD at one company I worked at came up with a pretty standard folder scheme, but her best idea was to prefix empty folders' names with --
This way you knew which folder was empty, that saved some time when looking for stuff.

i have very serious ocd(obsessive-compulsive disorder)..its hard to organize a perfect folder.

In that case, you should be very concerned that you didn’t capitalise The first I of either of your sentences, that you didn’t capitalise OCD, that you can’t small-caps OCD on QBN, that you missed the space between “d” and the open parenthesis and that your ellipses has only two dots.
Don’t talk to me about OCD!
