Its not an exact science to be sure. I've used the background eraser to some success. I've also been able to do it by copying the layer, desaturating, inverting, messing with levels and then making the selection on that layer and masking out the original layer.

It's a bugger, to be sure.
one quick'n'dirty method is to check the colour channels and see which, if any give you the greatest contrast.
Make a selection of that layer and use that and its inverse to colour-in or delete as required.
What I often do is strip back the hair as much as I can, then add it on afterwards with one of those follicle-shaped spray brushes they have in PS. Obviously that's no good for curly hair or bangs, or whatever.
Hmmm. I've just thought - try scanning some pubes, making them into brushes and using them as hair extensions for little Curly Sue.
