When invoicing a design agency for freelance work produced for them, what information should you include on the invoice? Do I really need to have GST/PST info on there or is it not relevant?

just put your tax id number so the gov't can track you(if you don't have one then use your social security number) and then your contact info and a description of services and a total. and the date.

If I'm going to be doing this more often do you think it's better to register with the government as a freelancer or just keep using my SSN?
My apartment complex forbids using the apartment as a place of work. Does this apply for a freelancer?
My employer has a 'Conflict of Interest' clause in their handbook but I don't have a contract. They say I should declare any outside work to them. Should I declare and risk them saying I can't do the work or should I just keep my mouth shut? Is there any way they can find out through tax records or anything like that?

if you're a freelancer use your SSN.
Check with your landlord, I guess they don't wanna have a lot of people coming in and out.I'd suggest you also talk to your employer, legally they can't tell u what u can or cannot do outside the office.
