Formmail help...?

I have set a questionnaire on my site, using formmail, all the info inputted into the site was being sent to me. But now it doesn't seem to work. Not sure if there's a problem with the coding?!

I'm not sure whether I received all of your emails? I tried doing it in php, but am really stuck. So i've gone back to formmail, just as a temp solution.

I really want to avoid the formmail form, but I'm not sure how to work with .php.
Are there any sites that explain just how to set up a simple php form, in the style of a questionnaire?
I found out yesterday that I can actually host php on my webspace. But i can't seem to upload any cgi script or anything onto the site, think this is to do with permissions.

Yep most basic hosting packages won't just let you dick about in the cgi bin. I know very little about php but I can just about send people emails through it. Loads of stuff about that you can use and read.

Can you point me in the right direction? I've typed into google 'setting up php forms' but it's bringing up sites which are just confusing me even more.
