According to my parents, I've been to various parts of the USA when I was younger. Can't remember anything though.
Other than that, I've been to Manitoba, Nova Scotia, Quebec, a bit of New York, aaand that's about it right now.

Spain, France, Italy, Greece
Awesome Itinerary.
Spain is amazing, France sucks, Italy is boring unless your with somebody else' GF in Florence, and Greece rocks esp the Islands, Thesloniki, and Halkithiki up North.
There you go, screw all the lonely planet guides, I've summed it all up in one sentence.

Indonesia - grew up and lived there for the most part of my life, loved it [and miss it]

Spain: Costa Brava
used to spend some summers there as a kid..my german grandparents retired there...loved it...christina beach and the towns of Tossa and Jerona are possibly teh nicest places i've been to.Mexico:
Playa Del Carmen
....heaven.....a tropical paradise...Germany: Goslar
lots of relatives there ..Gutersloh, Hanenklei, (those are places..not my relatives..haha) very nice...good people...very friendly....Amsterdam:
gorgeous city...amazing museums...and of course, the red light district and coffee houses...definitely a great trip, although after you hear teenagers from canada and the states talk about how awesome it is to be stoned, and how 'like dude..i was high this morning...and like...uh..im sooo high now...uh...wow..'
yeah...
you get the picture..
still tho..i whole heartedly recommend seeing this city...its great...Quebec City:
probably teh most picturesque city in Canada..certainly my favourite...old castle walls surrounding it....culture, great beer, cathedrals and churches everywhere, amazing ambieance...and fantastic people...
during the height of the referendum, when 'seperation' was shouted from every street corner, and it seemed like a bad idea to speak english where anyone could hear me, my wife took me to a pub downtown that happened to be basically GROUND ZERO of the seperatist movement...they were all singing proud nationalist quebec songs...i thought i was a goner if i opened my mouth...
then all of a sudden
the dude on stage started singing 'wish you were here' by Floyd...
in english..
and everyone sang along..
proving once again that music bridges all people..i totally fucking love quebec..
Halifax:

i was totally unprepared for how awesome a city this was..
incredibly friendly, a waterfront so nice it drives you crazy that Toronto's is so crappy..
amazing seafood...great live music...KEITHS brewery tour!

Japan: Amazing urban photography. Friendly people. Safe as hell. Treat westerners like movie stars (I had people wanting to take pictures with me when I landed at Narita :P ) Good sights. Climbed Mt. Fuji. GREAT night life..the asian women are amazing. Karaoke bars are innocent and just plain fun to get drunk at.
Vancouver, CA: Great walkable city. Urban in every sense. Very artsy/liberal. Great mix of cultures. Wanted to hit the night scene but never got around to it. Catch a ferry up to canada, tons of stuff leaves from the port here.
Those are the only places I would consider half-way exotic..other then that just all around the states ;)
