what is a SWOP approved color proof

  I'm doing an ad and I need to send SWOP approved color proof. They're also asking for a color bar, it says "Color Bar
  (6mm 5%, 25%, 50%, 75%, 100% CMYK Patches)
  How would I go about getting those?
