If they are silk or some really nice lace pattern, I typically will only hand wash them in the sink after wearing them a few times.

As far as I'm concerned the protocol is that you wash new underwear before wearing them.
That said, I also thought that the protocol is to always wear underwear and change them daily. If you are ok with not doing so, it's up to you mate.
