Does anyone know what should be included (besides a breakdown of hours spent) on a project? Should I include specific info on how many hours were spent on specific things (research, design, revisions)? Any iput would help.

i try to make the breakdown as transparent to the client as possible. the more transparent you can make, the better the clients understands what you through to do the work. keeping things as transparent as possible also means that it looks longer on paper and looks like you spent the time actually doing it. this leads to the client to think that what they're paying is worth the work you really did.

if they make a shitload of changes and crap that costs you time, then break it down for them to show them how much their annoying rediculous changes actually costs them! HA!

I've learned it's best to make everything very clear up front as to what you have agreed to do. Also I think it's important to look ahead and try to predict what the client may want in the future, that way you can make sure that you don't end up doing something for free down the road just because they decide they want something else which is something you could have included in the initial contract.
