I was wondering if anyone had thoughts on the best portfolio presentation for an interview. Most of my work is CD packaging and T-shirts, do you think it would be better to bring in the actually pieces, or make a booklet of images?

Most studios now prefer an online portfolio over a printed one, but there are still a few conservative agencies that demand a print piece.
That said, it is invaluable to have the actual work pieces with you. I would bring them to an interview upon request.

cd packaging - actual pieces
t shirt - booklet (plus one actual)

The booklet of T-shirts is a good idea! Those things are a pain to show.

wear like 10 of your t-shirts to the interview and keep taking them off every so often without saying anything.

doesnt hurt to have both a website and a book. just make sure they both show something a little different so they wont see the same stuff again.

if I were an employer, and you came into the interview doing what radar suggested...
I'd call the cops.

if yourr interviewing at a cool spot they would appreciate originality like that especially if they had been interviewing stiffs all day.
