Very simple
Just lay out rules
What typefaces are to be used when, and what weight/size/color etc
What Colors and icons are used etc
Also you incorporate branding guildelines and webpage grid dimensions
Menu treatments
margins etc....

I've written and read several over the years, but I don't have any online that I can show.
Think of it as a narrative that explains all the visual aspects and how they should be used, options, variations, etc. Use as many examples and illustrations as possible -- type treatments, colors, image dimensions, template usage.... the more examples you use the more useful the style guide will be.
Kinda of vague, hope this helps a little.

I can't find mine, but we have a visual explanation of one of our pages and what tag will dictate which css is used etc.
CSS makes style guides alot easier to write.

thanks guys, very useful stuff. i want to do it right, its for a huge client that is considering doing DOUBLE the amount of work with us in '05, so i want this thing to be stellar.

Yeah I write them everytime.
CSS guide with fonts & colours
and an IA guide with a wireframe of the layout
