Creating form

  I was wondering if you guys can help me out. I'm a print designer who was asked to create a form with the usual radio button, text field etc. I created all that in Dreamweaver MX. Now how do I get that info and process it to my email? What do I add under the ACTION tag and the CGI-Bin?
