How can I make a design span multiple pages, i.e creating an image that goes across two A4s thus creating an A3 sheet?

why not just create your art board the size of the A3 page?
If it's a spread, just use a verticle guideline so you know where the gutter is.

Easy.
File > Document Setup
Create a 420mm wide document (2 x A4 Portrait)then you need to tile your pages using the page tiling you're not actually going to print at this point, but you're going to tell the printer what's going on.
File > Print
Setup (depends on printer, system) Tile Full Pages.Then you can use your "page tool" which is in the set of tools under the "hand tool" to set your new page tiling to the proper area.
So then you should have a 420mm x 297mm artboard with the page tiling on it with a 1 and 2 in the bottom left hand corner of those.
When you go to print you can have it print the consecutive pages for your page tiling and it will automatically cut your image appropriately. Though this will probably only work for full bleed.
