form submission

  gotta a little question about how to execute this form submission correctly.

  Would like to have a visitor fill out an HTML (or flash embedded form) that has them submit some text info and an image from their harddrive.

  The form to send this to a specified email address.

  Anyone know of a script I can learn from, or offer a solution?

  You can use CGI's or a server scripting language like PHP or ASP to do this. Maybe you have an available CGI with your hosting account?
