client questionaire

i am trying to create some sort of questionaire for my current and past client to fill out for some good feedback...
questions like: how was your experience with the designer, what cwould you like to see operationally done better, review cycles, was I professional enough, etc...
has anyone ever done something like this? Is so, or not, any input would be appreciated.

something like that... being that this is all freelance, some of my clients use some pretty big agencies for projects as well. I want to know what I do differently...na meen

i've got something similar in print - it's actually for contracted employees, but some questions apply.

hey lyrek, i work fulltime as an art director now and freelace on the side. i have been hesitating to go fulltime freelance until I can make the same $$ or more with a more steady stream or work.

well, we should be heading out on thursday again - u missed reggae night, punk.
Friday, the old owner of McStones (close to your day job) is having a reunion party for all us drunken regulars. I'll probably head down there for that.
By the weekend I'll be too tapped out and hung over to hit a crawl though.
