anakin has a duel with obi wan on a volcanic planet, and gets cocky. obi wan has the high ground and warns him 'i have the high ground, it's over', but anakin believes his powers are greater and tries to jump over obi-wan. obi-wan cuts off both legs and his one good arm with one move, leaving anakin a carcass with only his mechanical arm. then some lava spills on him and he catches fire. obi wan walks away and leaves him for dead.
of course the emperor has been feeling he's been in trouble and went to fetch him.
bunch of robot doctors repair him and slap the darth suit on (without any kind of healing or therapy, just straight into the suit he goes). then, in total frankenstein moment he rips himself off the table when the emperor informs him that he supposedly choked padme to death earlier on the lava planet.
padme in the meantime is on the delivery table, saved by obi-wan, has her two children but has lost the will to live because anakin has clearly gone insane. so she dies, and the kids get sent to alderaan and tattoine respectively.
