nice, but:
has the weird usability thing of 90° offset btw front and back...
would tweak photo a little for his shirt to better match the exact hue of the front...
the absence of state is odd when all other info is extra detailed and clear
colons in type could just go, it seems clear enough
kern the 1 in the phone numbers closer to the rest of the numbers, the space is a little odd.
