Don´t blame nobody.
Keep up with the positive attitude and clearly state this experience proves something didn´t work, and can be used to define a change in future communication via e-mail. For meetings or whatever.
Get something out from this. Blame is a waste of energy.
Be postive and re focus the issue in what can be improved and you´ll be fine.
