It is usually due to a bacterial thing... it is quite repulsive the weird scale thing...
the fish is probably doomed!
I was treating my fish for something a few years back... and came back from a night out, and put the 20 drops of ointment in the water... but coz i was pissed, each drop was slightly too big... so overdosed them! Spent the whole of the next day trying to rescue the remaining ones by swapping the water.... nightmare... fish are probably one of the hardest things to look after.
