why don't you just cut it to him straight, like "why are you acting like a bitch all of the time? what makes you think as an intern you are above certain medial tasks?"
maybe don't use the word 'bitch', but just be real with the kid. ask him honestly what the deal is.
i think that's probably the best thing to do, although i like these other guys suggestions too.
