  There are tonnes and tonnes out there, just look around. I recall a thread a week or so asking for the same thing. Also try http://www.hotscripts.com and similar sites.
  And for what it's worth, PHP or Perl form scripts are really easy to write - probably the second thing you learn after sending "Hello World" to the browser.
