    I get asked this all the time, and it's one of The Reaons I really should get myself a small but powerful CNC mill.

    If you're ok to 3d print them, try and go for a resin printer - they're actually quite cheap to buy these days, for about the dimensions that you're looking for - I think imbecile's got a recommendation in the 3d Printing thread here.

    Other options are to buy metal discs and chemical etch them - still fiddly as you'd need to do them in two passes (front, then back) and likely have a laser-etcher describe the design through a black mask (as I imagine hydro's coins above might have been done?)

    Failing that, a laser that can work on metals directly, but the sort that can do tangible physical relief are out of my sphere of understanding, unfortunately for me.
