I used to be a graphic designer, years ago, but moved on to advertising, where I spent many more years as an art director, and now a CD since the last few years.
I've found the simplest way to explain it is to simply say to people, 'I'm an Art Director by trade, working as a Creative Director.' It gives people both enough background to figure out your background, whilst still expressing the natural evolution.
