I normally give a set price per illustration- normally for me that's about 5-7 hours each, including concept sketching and finishing- but that time will be different for you based on detail and experience.
I usually have a set rate for each revision- usually about 75% of the original, as revisions usually dont require as many concept sketches and such.
I also usually quote the design/layout as 40 hrs, with additional time in blocks of 5-8 hrs to be paid in advance as needed.
you'll also want to make sure they're paying for an isbn and loc- just so nobody runs off with your work.
these are just a few guidelines i have for myself and to help make things clear with the client. It happens every time that an illustration is finished off and then they say "i like it, but can we have he holding a glass of wine" - like it's just a magic button that does it. If they know they're paying for the wine, they're more likely to tell you up front or not be bothered with it at all.
