I think turnerworks is right, as far as I know you can't apply an anchored object to a paragraph or character style as it's basically a special text character. You're probably gonna have to add the image to your answer sections manually :(
You can set up object styles to control the appearance of the anchored object/image, but inserting the image is probably gonna have to be done by you and your fingers.
That nested styles shit is a little bit over my head (though I am slightly drunk) so I won't even try to help with the rules thing.
