Object > Anchored object > Insert for the image part. I don't think you can apply that to a paragraph or character style, but if you copy and paste at least it will stay inline with your copy as you edit it.
You could use nested styles to create a first paragraph without a rule and all subsequent paragraphs to have a rule... or a character style you apply to either the top paragraph to remove a rule, or the rest to apply a rule. If you're linking your text boxes there's no way to tell InDesign to detect a paragraph that's at the top of a column.
I know you don't want to use another style, but the only other way is to manually remove the rule by selecting said paragraph and using Cmd+Option+J (or the paragraph panel flyout menu) to open the paragraph rules and turn it off there.
