you should create an indesign doc with 2 pages - you can do separate docs for interior/exterior if you want - with guides / lines that define the fold (make sure you specify the color as 'GUIDES - DO NOT PRINT').
Then just design accordingly. That's it. Any printer who's not a complete idiot should know what to do with your file.
But about your question... uh.. tried and trusted? Well.. you can format your doc anyway you want to. Are you trying to print yourself or send to a printer?
Oh also, if sending to a printer, why not just ask them the best way to format a document and if they can recommend a 'generic' template/setup for the trifold?
