I had a real problem like this before. It was depressing and it sounds like you are burning yourself out.... I run the risk of repeating what the others have said but here it is anyway:
I stopped smoking weed for a month (which turned into a year when I realised how much of my brain I got back).
I set myself a tea break at the same time every day which got me away from my desk and walking around... if i could sit outside for 15 min I would. Fresh air helps.
Always did my best to take an hour for lunch.I used to get annoyed that I couldn't get as many nights to myself so I stayed up later... I forced myself to bed at a reasonable hour and got up a little earlier too (to try to tire myself out a bit), which turned out that I needed more exercise, which really helps, even go running for 30 min every other day will help.
