i've taken to using apple ProRes for everything. i get lots of mixed sources with different codecs but as long as i set my sequence to use apple ProRes, my final exported self contained movie is ProRes. for me that codec is the best quality for the smallest file size.
as for exporting out FLVs, i'm using adobe media encoder that came bundled with the CS4 package.
