Advertising / fashion - AD generally dont design. They provide high concept. General theme, mood or story.
Design shop - AD do design. They set the tone of the design and designers follow direction.
Advertising / fashion - VD only design. Production pretty much. Execute AD's idea.
Design shop - VD can be AD or there is no AD. VD can provide more detailed solution.
Shop like IDEO - VD can be same level as say ACD. Different track of thinking and mode of expertise.
