every single point you're asking will get every range of answer here.
from my point of view:
• find the market rate per square metre for a base rental cost
• work out the bills and either include an average with the rental amount or simply make that a transparent shared cost
• if you're offering a printer / copier then that could be extra - or a bonus if you meet someone you'd like in there
• feel / look is important but from my experience beggers can't be choosers so it's secondary to where and how big the space is.
