It's only about 1/4th photoshop, but 3/4th professional lighting and exposure, things aligned very carefully. Photographer often has many lights, with careful placement, some diffused, some snooted. Careful time is spent testing with stand-ins, then the talent shows up and the actual job doesn't take much time.
